We provide member forms as:

1. Standard PDF - to print, complete and send.

2. Editable PDF* - complete the form online, print, sign and send.

*We need an original signature(s) for all forms.

Click on the link for the form you wish to use. This will open a blank form.
Immediately click on ‘Save’ before entering any information and save the blank form in an appropriate folder.
Open the saved form, complete form information and re-save (with appropriate file).
You can then over type on this form and re-save under a different file name if necessary. If you see information you have typed on a previous occasion, please ignore and type over.

LGPS RET 2 - (Standard)	Use this form where retirement (not ill health) has been discussed with an employee. The details will be used to provide, direct to the employee’s home address, a provisional estimate of benefits and a pre-retirement pack giving details of the various options available.
